I have version 4.9.1.10 (Mac)
After some testing, I believe Download Manager does not work properly.
For example, I tried to install Websters. It is not available in BA after a restart.
I did something else. I deleted the Wesley Notes from the directory. I restarted, the Notes were not available anymore. I reinstalled by the Download Manager, same result: not seen. I recovered the file and it is available in BA.
BTW, I have wesley.cmt and wesley-notes.cmt in my directory. Perhaps a list of all the files available with extensions can be published?

I think this may be a permissions issue with the folder,
/Library/Application Support/Bible Analyzer/Modules
(This folder is now hidden in OS X)
Check and see if this folder (and all below) has both read and write permissions.
Because of some changes in permissions, etc. we may have to move all the modules to the USER's folder. However, there only that specific user will have access to them.

Sorry, permissions were all set, the problems persist here.

I just uploaded an update for Bible Analyzer, 4.9.2. It fixes some other issues and I added the option to select an additional folder path for modules. See Options I in Preferences.

But I still don't know why some Macs seem to have an issue installing mods with the Download Manager. I cannot reproduce the problem. I'm on 10.6 and I hesitate to update to 10.9 because many Mac users haven't and still run older versions.

I see some progress
But, the BI OT & NT and the Hymns Bundle are not installed correctly.
The download module keeps downloading them.
Where are the files located and how are they called?

On Mac the files are in the folder
/Library/Application Support/Bible Analyzer/Modules
(This folder can be found by clicking Tools/Display Primary Modules Folder in the Menu)
The Download Manager looks for a file named "bi-ot.cmt" in the /Commentary folder to check if the Biblical Illust. is installed.
For the hymns it looks for "Rock of Ages.mid" in the /Audio folder

I had the file bi-ot.cmt and bi-nt.cmt, but in capital letters. I removed these files and now download manager works well. It does not re-download these files.
But regarding the mid files, I deleted these but download manager keeps downloading them.
