There are two things needed in your file. You can create it in ScripturePad or any plain text editor formatted as you like and then simply delete the line breaks when you are done. You can use Notepad++ and search for \r\n (in RegEx mode) to remove them. Just replace with a space to keep words from running together. If your work is only one...

Vine's sometimes has multiple entries for a word. The two instances you mention are demonstrate that. As for "cross," there is also an entry called "cross, crucify" which has the entry you seek. These are OCR errors that can found found on occasion. That is how most of these older titles came to be digitized. Since these titles are often free or at very lost cost there are no funds available to pay a proofreader.
